Acrobat User Community Forums

I am creating a form using Adobe Acrobat 8 Professional.  I would like to add one drop-down box that lists 10 different store names and their corresponding addresses. Another option that may work is if I can show the user the different store names; when the person selects a particular store, it automatically completes the store's address. How can I do this?
