Hi ramnymdpa,

You don't mention what process you're using to convert your Word form to PDF so I'm assuming that you're using the "Run Form Field Recognition" command in Acrobat 8. If so, this process does not always capture checkboxes. You'll need to add this back in with the Acrobat Forms tools.
