I have Acrobat 8.1 Standard--and no toolbar in Outlook 2003. Used the repair tool and still no toolbar. TIA

Sounds like perhaps your PDFMaker toolbar was put on the Disable Items list. Try these steps:
1) Open Outlook 2003 program.
2) Go to Help > About Microsoft Outlook.
3) Click Disabled Items.
4) Select Adobe PDF from the list and click Enable.
5) Quit Outlook 2003 then restart it.
