I have designed a form with an e-mail button which works but when the user returns it using the e-mail button the form isn't there only the data. How can I change it so the form w/ the information is returned?

If the user is using Adobe Reader to fill in and submit the form then you will need to license Adobe LiveCycle Reader Extensions ES to enable the form to submit more than simply the data.
If these forms are for an audience of less than 500 individuals then you can create your form with Acrobat and enable the return features from the Forms options.
