I have a form with three columns and seven rows, the user is to mark a check mark next to the appropriate box, but when one box is marked, all 21 boxes are marked with a check mark. How can I mark only the boxes i choose?

Each check box needs to have a different name. In the properties dialog box, on the general tab, change name to something different for each check box.
