Filliable PDF form with link to database

Hi
I hope someone can answer my question, I would like to know which software I need to create a pdf Form filliable but at the same time some of the fields are linked to a database where I can select wich ones will be printed.
Also if there is a tutorial how to do it.
Please I'm really confuse with all the names adobe reader, distiller, livecycle

You did not mention the version of Acrobat you're using.

If it is version 7 or below, you can use ADBC (Adobe Database Conectivity).
