You upload a PDF document to your Web server just like you upload images and HTML files. Use an ftp client to upload the files.
You add a Submit button by creating a Button field and in the Actions tab in the Field Properties, choose Submit a form and then click Add. A dialog box opens. At the top of the dialog box in the first field type: mailto:email address. Don't forget to add mailto with a colon, then type the email address you want the user to send back to you.
If your returned forms are less than 500, be sure to enable the form with Adobe Reader usage rights and the Reader user can submit the entire PDF form back to you as well as save the field data on their end.
